1. Call Association Member Benefits Advisors (AMBA) (formerly Mercer Consumer) -- the plan administrator -- toll-free at 888-386-9788 weekdays from 9 a.m. to 7 p.m. (EST).

Online applications for the MetLife Voluntary Disability Insurance Plan are available on the Disability Insurance page on the AMBA website; click on the "Apply Now" blue box on the right hand side of the page.

NYSUT members can also print out an application if they would prefer by clicking on the Disability Insurance page on the AMBA website; you will then need to click on "Disability Application" under "Disability Forms" on the right hand side of the page.

Applications must be printed out, completed, and then mailed to AMBA, P.O. Box 14522, Des Moines, IA 50306-3522.


The NYSUT Member Benefits Trust-endorsed Disability Insurance Plan is underwritten by Metropolitan Life Insurance Company and administered by Association Member Benefits Advisors.

NYSUT Associate Members and NYSUT Student Members are not eligible to enroll in this program.

The Metropolitan Life Insurance Company Disability Plan is a NYSUT Member Benefits Trust (Member Benefits)-endorsed program. Member Benefits has an endorsement arrangement of 5% of gross premiums for this program. All such payments to Member Benefits are used solely to defray the costs of administering its various programs and, where appropriate, to enhance them. The Insurer pools the premiums of Member Benefits participants who are insured for the purposes of determining premium rates and accounting. Coverage outside of this plan may have rates and terms that are not the same as those obtainable through Member Benefits. The Insurer or Member Benefits may hold premium reserves that may be used to offset rate increases and/or fund such other expenses related to the plan as determined appropriate by Member Benefits.
