By submitting this enrollment form, I waive my attorney/client privilege only with respect to the processing of record-keeping information relating to the Plan, and not with respect to the substantive content of any legal matters covered by the Plan. Due to batch processing, your credit card may or may not be charged on the same day you submit your online enrollment form. Once you submit your application online, your NYSUT membership is verified, the application is processed and your credit card is then charged. Your fulfillment package will be generated and sent within 14 - 21 business days from the online submission date.
